Questions for the recipient

These are candidate records and questions for the selected purpose, not a universal required-document list or an insurance-payment rule.

Requester-route documents

This prepares an existing-record access/copy request; it is not an approval decision.

  1. A means of confirming that you are the patient; ask the institution which identification it accepts.

Ask the receiving organization about the record range, language and original-or-copy requirement for your purpose. First issuance of a diagnosis certificate or opinion and insurance payment are outside this list.

Enforcement Rule Article 13-3 and Annex 2-2, current 12 June 2026. Consent Form 9-2 revised 20 June 2025; authorization Form 9-3 revised 7 November 2024.

Inputs and selections are processed only on this screen. They are not sent to a server or saved in the address or browser storage. Do not enter names, identity numbers or medical-record files.

What does the record-request checklist prepare?

It lists documents for an existing-record request by requester route; it does not decide approval or first certificate issuance.

Choose the purpose and requester route without entering a name, identification number or record. Purpose changes the recipient questions and candidate material; the requester route determines the legal preparation list.

Purpose gives context but does not change the legal document list. A receiving institution can clarify the record range and language it needs.

For example, selecting a patient request gives an identity-confirmation item rather than demanding one particular identification copy from every patient. Article 13-3(4) establishes the confirmation duty. Selecting a relative instead introduces proof of the qualifying relationship and the applicable consent requirement. Selecting a designated agent introduces authorization and the patient-identification rule. These examples show why a request purpose alone cannot choose the document list. An insurance purpose does not turn an agent into a qualifying relative or remove identity checks.

How does the record-request checklist distinguish age conditions?

Under 14 consent rules differ from the designated-agent patient-ID exception for a person under 17 without an issued resident-registration card.

For a qualifying relative, the current paragraph does not list the patient’s identification copy. For a designated agent, the patient-copy rule remains subject to its stated exception. The legal representative completes the consent and authorization documents for a patient under 14 on the agent route.

For a relative requesting records of a patient under 14, the consent-document exception belongs to that relative paragraph. For an agent requesting records of a patient under 14, the legal representative completes the consent and authorization and attaches proof of representative status. For a patient aged 14 to 16, the agent route still uses the patient’s signatures; the patient-ID-copy exception depends on the resident-registration card not having been issued. At 17 or older, the under-17 exception cannot be selected. These are separate document conditions, not one general minor exemption.

Qualifying relativeRequester ID, kinship, consentConsent exception for under 14; sibling condition
Designated agentRequester ID, consent, authorization, patient IDUnder 14 representative; specific under 17 ID exception

Why can the record-request checklist show an unresolved condition?

A sibling route requires specified other relatives to be absent; other relationships require the applicable legal route to be clarified.

Warnings appear before the list so the list is not mistaken for a completed eligibility check.

For death or the specified inability-to-consent situations, the annex evidence applies. These situations do not assume that the patient can sign a new authorization form.

For a deceased patient, the listed situation evidence can include a death certificate or the specified family-register material. Unconsciousness or severe illness/injury preventing a hand written signature uses the confirming diagnosis certificate described in the annex. Missing status and inability to make decisions have their own listed evidence. These are not interchangeable reasons for skipping consent. The guide preserves the annex’s terminology and never diagnoses the patient’s capacity. A sibling also needs the absence condition even where one of these situations is selected.

Which forms support the record-request checklist?

Read current Rule 13-3, Annex 2-2, Consent Form 9-2 and Authorization Form 9-3 for their respective functions.

The consent form was revised 20 June 2025 and the authorization form 7 November 2024. The rule edition checked here is effective 12 June 2026.

Relative identity alternatives are described in Article 13-3(5); the institution confirms its intake procedure.

The consent form lets the patient select access, copies or transmission to another institution and identify the requested range. The authorization form identifies the agent and delegated request. An old downloaded form should therefore be compared with the current linked version rather than assumed current because its title is familiar. Consent and authorization have different functions. The rule’s relative identity alternatives also do not remove relationship evidence or transform the designated-agent document list into the relative list.

Are inputs to the record-request checklist sent or stored?

The selections are processed on this screen and are not sent, stored in the browser or added to the URL.

Only general categories are selected. The result is a preparation list, not a submitted request.

Submit actual required documents through the issuing institution’s accepted process rather than through this guide.

Changing a selection updates the preparation list without submitting a request to an institution. No form fields for a name, birth date, address or identification number are presented. Although official downloadable forms contain personal-information fields, those fields are not copied into this tool. Keep the actual completed forms and records within the issuing institution’s accepted process. The screen result is not a document-receipt notice, a signature check or evidence that an institution has approved the requester.

What are the limits of this tool?

  • Preparation information only; check the issuing institution’s actual procedure.
  • No patient files or identity details are needed.

Original scope, dates and limits remain attached to each source. Missing evidence is not replaced by assumptions.
